Dr. Faith Foreman-Hays, Assistant Director with Houston Health Department and chair of the Core Team for Cities Changing Diabetes - Houston will kick off the event.

Next, Dr. Hope Galvan who leads population health at Harris Health System will offer her perspectives on opportunities for multi-sector collaboration to address health equity in diabetes prevention and care.

Moderated by Tanweer Kaleemullah, program officer at Episcopal Health Foundation, panel members representing patient, healthcare professional, CHW, and community voices will offer their solutions to improve the lives of Houstonians living with diabetes.

At the end of the program, Stuart Nelson, Acting President of the Institute for Spirituality and Health, will provide you with updates of activities in Cities Changing Diabetes, including the MyDiabetesHQ.org resource and navigation hub currently in its final stages of testing.
